    Important:

    1. If your child has been immunized out of the country, out of province, out of VCH health authority or by your family physician or pharmacist we may not have the record of those immunizations. Please email your child’s translated record (into English) at immsrph@vch.ca as soon as you book the appointment and bring these records to the appointment

Without a current immunization history, we may need to use your appointment time for planning and may need to book a separate appointment to complete the immunizations.

    2. Please book the appointment using:
      • Child's legal name
      • Personal health number (PHN) if available.
    3. Please book one appointment time for each child attending. For siblings or family groups, please book appointments following one another if possible (e.g. 9 am, 9:30 am).
