We specialize in providing compassionate care and comprehensive assessments tailored to the unique needs of older adults. At our clinic, we understand the importance of maintaining health, independence, and quality of life as we age.
Our services include thorough geriatric assessments conducted by experienced geriatric specialists. These assessments are for seniors over the age of 65 years, and encompass a holistic approach, addressing medical, functional, cognitive, and psychosocial aspects of aging. We believe in personalized care plans that are designed to optimize each individual's well-being and support their specific health goals.
In addition to our assessment services, we offer expert physiotherapy tailored to the needs of our clients. Our physiotherapists are dedicated to enhancing mobility, strength, and overall physical function, helping older adults maintain or regain their independence and active lifestyles.
We also offer a teaching clinic which provides opportunities for medical students, residents and fellows to see patients under the supervision of a geriatrician.
How to access services

Eligibility
Our program is intended for patients 65 years old or older and suffering from dementia, delirium, disruptive behaviour, and failure to thrive as well as those with increased risks of institutionalization.
These issues include:
- Cognitive impairment
- Multiple medical comorbidities and frailty
- Functional decline (ADLS, IADLS concerns)
- Pre-operative assessment
- Mobility concerns and fall assessment
- Delirium
- Medication review/polypharmacy
- Behaviour/mood concerns
- Movement disorders (Parkinsonism, etc.)
- Syncope, fainting
- Living at risk/failure to thrive
- Competency assessment

Please have a health-care practitioner fill and submit the Geriatric Medicine referral form.

- Monday: 8:00 a.m. to 4:00 p.m.
- Tuesday: 8:00 a.m. to 4:00 p.m.
- Wednesday: 8:00 a.m. to 4:00 p.m.
- Thursday: 8:00 a.m. to 4:00 p.m.
- Friday: 8:00 a.m. to 4:00 p.m.
- Saturday: Closed
- Sunday: Closed
