Home Health Dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).

How to access

  • Check eligibility

    Home Health Dietitians work with clients who are unable to leave their homes. The community health team may refer clients to the ambulatory Home Heal Dietitian if they can and are willing to be seen at the community health centre.

    All nutrition services are free of charge.

  • Get a referral

    ​If you are already a client of a community health team, please speak to your Vancouver Coastal health contact and ask to be referred to a Home Health Dietitian.

    External referrals can be made through your primary care provider or by calling Central Intake at or (604) 287-3361.

The clinic is closed on weekends and statutory holidays. Hours may vary for specific services or programs, please refer to the specific service or program to confirm hours.

Parking and transportation

Parking access to the parkade driveway is only available when driving East on Granville Avenue. 

Pay parking is available on 1st two levels of the parkade – pay machine on 1st level.
