The Intensive Case Management (ICM) Team provides a support role for people who experience severe substance use problems, chronic unmet medical needs, and multiple barriers to accessing services.

- Residents of Squamish aged 19 and over with problematic substance use concerns

- Doctor or nurse practitioner referrals are also accepted. If you are a physician or nurse practitioner looking to refer, please fax a completed Sea to Sky Referral Form to 604-815-4197.
