The North Shore Palliative Outpatient Clinic offers services for patients registered with the North Shore Palliative Care Program who require additional support managing complicated pain and symptoms or end-of-life/palliative planning. Appointments are provided by a palliative physician or nurse practitioner.

Outpatient Clinic appointments are not meant to replace routine medical care performed by the patient’s primary care provider.

How to access

  • Get a referral

    Patients not registered with the North Shore Palliative Care Program must have a referral placed by their physician (Family Doctor or Specialist).

    Patients registered with the North Shore Palliative Care Program can request an appointment by calling the Palliative Program Assistant at (604) 984-3743.

How to get here

Driving North on Lonsdale

Turn right at 13th Street and pass Lions Gate Hospital on your left. The Hospice entrance is located on the corner of St. Andrews and 13th on the left. 

Driving South on Lonsdale (from the Upper Levels Highway)

Take Exit 18 to Lonsdale Avenue South and proceed to Lonsdale Avenue South. Turn left at 13th Street and pass Lions Gate Hospital on your left. The Hospice entrance is located on the corner of St. Andrews and 13th on the left. 

Parking

Parking is available at the rear of the hospice – please visit the nursing station for more information. There is limited free parking on the surrounding streets.

North Shore Hospice

The North Shore Hospice is an option for those in the last few months of life and home is not an option due to care needs, symptom management needs or patient and family goals.

Palliative Care through Outpatient Clinics and Programs

For people who can leave their homes, North Shore, Vancouver, and Richmond offer a range of Specialized Palliative Care Services and supports for clients and their family members, caregivers and/or bereaved through outpatient/ambulatory programs and care settings.
