The RESPONSe project team exemplifies exceptional performance by addressing a critical gap in the ability to provide timely symptom management during cancer treatment.
Understanding that patient-reported outcome measures in oncology can enhance communication and quality of life and reduce symptom burden and acute care use, the clinicians collaborated with Virtual Health and VCHRI on this project.
This program has transformed symptom management for cancer patients through innovative remote monitoring and proactive care. Through integrating digital symptom monitoring and a dedicated Symptom Management Nurse, the program has enabled timely interventions based on patient-reported outcomes. Since launching in January 2024, the program has significantly reduced emergency visits, increased patient satisfaction, and improved communication between patients and clinicians.

The Palliative Specialty Nurse in the emergency department (ED) project team addresses a gap in care for adults with serious illness or life-limiting conditions and their end-of-life care.
As the population ages and becomes increasingly medically complex, many patients in their last months of life are presenting to EDs, often in the final stages of serious illness. However, the ED remains a challenging place to provide a palliative approach to care.
This project, piloted at Vancouver General Hospital, embedded a Palliative Clinical Nurse Specialist in the ED to proactively identify patients with palliative care needs, facilitating goals-of-care conversations and connecting patients to appropriate supports.
The Clinical Nurse Specialist has helped prevent 22 acute admissions, while improving patient-centred care and reducing ED stays.
