If you received care at any of our locations, you can request copies of your medical record or health information.
Everyone treated at one of our facilities has a health record. These records contain reports from care providers, test results, treatments, and pharmacy information. Health records can be paper based, electronically generated or a combination of both.
HIMconnect.ca provides centralized health information resources for patients, health-care providers, researchers and institutions across the Lower Mainland, Vancouver Island and the B.C. Okanagan.
How do I request my health records?
You or anyone authorized to act on your behalf can request access to or copies of your health records by completing the Authorization for the Release of Health Records form.
If you are requesting your own health records, you can submit the request using this online form or by using the PDF form below. If you are requesting someone else’s health records or if someone is authorized to act on your behalf, the PDF form must be used.
Submit the form by email, mail, fax or in person to the Health Record Department at the location where you received care. A list of locations is available on HIMconnect They will assist you in accessing the records you wish to see.

How long will it take to obtain copies of my health record?
You will receive a response within 30 business days from the date we receive your written request.

Is there a fee for requesting health records?
Generally no, but you may be charged for special processing of your request. For example, courier fees, special delivery, or other special processing services.

Immunization records
BC residents can access their immunization records online using Health Gateway. It is accessible by anyone 12 and older with the BC Services Card.
Health Gateway only shows immunizations in the Provincial Immunization Registry; this includes immunizations given at public health clinics and pharmacies in B.C. It does not show immunizations given by other health care providers (i.e. family doctors/nurse practitioners).
