Many people prefer to spend their final days at home, surrounded by familiar faces and in a comfortable environment. We support this choice by providing care at home and helping caregivers, family and friends.
We offer a range of supports and services that are available in the home throughout a person’s illness journey to maximize comfort and quality of life.
This includes providing care at home before, during and after death for clients nearing their end-of-life and for their loved ones.
How to access
We can answer questions about eligibility and help determine the best care option for you. Once you have been assessed, we will help with the application process. Contact us for more information or to request an assessment.
What to expect
To receive palliative or end-of-life care in your home, you must be connected to Home Health. Once connected to Home Health, discuss your goals, wishes, and values with your care team. Your care team will help you arrange the right services for you.
Depending on your care needs, your care team may recommend one or more of the following services:
- 24/7 at-home nursing care
- 24/7 at-home respite for families/caregivers
- Equipment or supplies in the home
- Community outpatient palliative care clinics or wellness programs
- Hospice for short-term respite care or end-of-life care
- Spiritual Care services
- Grief/Bereavement support
Note: People registered with BC Palliative Care Benefits may be eligible to receive most of these services at no cost.
