Ang Youth Intensive Case Management Team (ICMT) ay isang multidisciplinary team at ang layunin nito ay ang mag-alok sa kabataang may mga komplikadong pangangailangan (ibig sabihin, paggamit ng droga o alak, homeless, may mga paghahamon sa mental health) ng innovative na mga serbisyong nagbibigay-diin sa kliyente at na sensitibo sa kanyang kultura.
Ang mga layunin nito ay ang karagdagang patuloy na pangangalaga at pagsali sa kabataan sa kanyang sariling pangangalaga, at mas mahusay na komunikasyon sa pagitan ng mga miyembro ng care team ng isang kabataan.
Kabilang sa mga serbisyong inaalok:
- access sa primary care,
- access sa Opiate Agonist Therapy,
- kultural at spiritwal na suporta at mga serbisyo,
- outreach sa komunidad,
- mga referral at tulong sa pagkonekta sa mga serbisyo, kabilang na ang mga pangmatagalang suporta sa komunidad,
- harm reduction supplies at edukasyon,
- Occupational Therapist (OT) assessment at suporta,
- pagpaplano ng treatment, at
- koneksyon sa pro-social supports.

Ang mga layunin ng Youth ICMT ay ang karagdagang patuloy na pangangalaga at pagsali sa kabataan sa kanyang sariling pangangalaga, at mas mahusay na komunikasyon sa pagitan ng mga miyembro ng care team ng isang kabataan.
Kabilang sa ICMT Team ang:
- team leader,
- intake clinicians,
- case managers/clinicians,
- youth outreach workers,
- cultural worker,
- Nurse Practitioners,
- Social Worker, at
- Occupational Therapist.

Transition into Independence (TIP)
The team also encompasses a housing subsidy program called Transition into Independence (TIP) which offers a monthly housing subsidy for market housing for VCH catchment area youths ages 18 to 25. Youth are connected with a youth care worker providing case management and intensive support to youths. Youths can also be connected to primary care, cultural and spiritual support, case management or other community or treatment supports.
The Transition Group
The team also offers a life skills group named the Transition group. The Transitions Group gives youth who are transitioning into adulthood the opportunity to learn life skills in a supportive, low-barrier environment along with Vancouver Coastal Health.
Participants are between the ages of 17 and 24 and have had some involvement with MCFD or VACFSS.
Examples of topics covered include:
- healthy eating,
- eating on a budget,
- income and identification,
- education and employment, and
- physical and mental health.
