北岸青少年飲食障礙計劃 (The North Shore Youth Eating Disorders Program, NSY EDP) 與小兒科醫生合作，在臨床環境中提供醫療監測，並為患有飲食習慣不良／臨床飲食障礙的青少年提供輔導和營養支援。

如何獲取服務

  • 查看是否符合資格

    NSYEDP 為 12 至 19 歲患有神經性厭食症、神經性暴食症或其他特定／未特定飲食障礙症（舊稱為「未明示之飲食障礙」）的使用者提供服務。

    服務使用者患有：

    • 對身形和體重有強烈關注和擔憂，以及
    • 由於自願性限制食物攝取而導致體重顯著過輕或減輕。 或
    • 暴食伴隨著失控感（即無法停止暴食或控制食物攝取量）和清除行為（即催吐、瀉藥、暴食後禁食、過度運動）。

    我們的小兒科醫生將對 17 歲以下的青少年進行評估。如有必要，18 - 19 歲的青少年也可經由 Foundry 醫護人員進行評估。我們計劃中的所有青少年都需要定期向聯繫他們的初級醫護提供者，進行醫療複診。

  • 獲取轉介

    需要初級醫護提供者的轉診。將完成的轉介表格傳真至 Foundry (604) 984-5061 。

It is an integrated youth and family initiative that will transform how mental health and substance use services are delivered on the North Shore, providing early intervention to support young people's well-being.
