Evidence suggests that returning home as soon as safely possible and continuing your recovery in the familiarity of your own home is often better than waiting in the hospital to fully regain your strength.
Many services and supports are available in the community to help patients continue their recovery and regain strength and independence. As necessary, a community clinician will visit you in the privacy of your home or see you in a clinic setting to treat and assess ongoing care needs.
In following the Home is Best philosophy, it is important to us that patients and their families are well-supported at home. If, eventually, you can no longer safely remain at home, we will assist with transitioning you to the most appropriate care setting.
Our allied health team will complete their assessments and in connection with the patient, family, Care Management Leader and Transition Services Team member will set up an individualized care plan to ensure you can safely be discharged home.
UBC Hospital Complex Activation Medical Unit (CAMU)
The allied health team may also consider the possibility of an activation unit at UBC Hospital called the Complex Activation Medical Unit (CAMU).
This unit is an inpatient unit for patients who no longer need the specialized medical services at VGH and only require a bit more time, average stay is 2 to 3 weeks, to work with physiotherapists and occupational therapists to gain the independence to return home or achieve the necessary activity tolerance to be referred for intensive rehab.

Some home and community care services are provided free of charge. Other services include a fee, which is determined by your income.
Stroke Prevention Clinic
All patients will be referred to the stroke prevention clinic after discharge from hospital. The medical office assistant will contact you for an appointment: the appointment is usually 6 weeks after discharge.
